Why didn't my international order ship?

In an effort to reduce fraudulent orders, ALL international orders will be placed on hold until shipping costs are approved by the customer and a cardholder verification form has been submitted, reviewed, and approved. ALL first-time orders must be shipped to the billing address. No exceptions will be made! We reserve the right to cancel any order with fraudulent characteristics.
